Download the correct file from playstation.com
Use the official PS4 system software page. For a normal update choose the update file; only use the larger reinstallation file if you intend to wipe and reinstall.

Format the USB stick as FAT32 (or exFAT)
On the drive, create a folder named PS4, and inside it a folder named UPDATE. Save the file as PS4UPDATE.PUP — the path must be exactly PS4/UPDATE/PS4UPDATE.PUP.

Re-copy the file fully
Let the copy finish completely before ejecting — a half-written .PUP file is the most common cause of this rejection.

Try a different USB drive
Some drives or ports aren’t detected reliably. A smaller, plainly formatted stick in a rear port works best.

If updating normally, just retry over the internet
If you weren’t using USB, the downloaded file may be corrupt — go to Settings > System Software Update and let it re-download.

Signs the problem is on Sony's side — or in the hardware — rather than anything you can change:
- The same correctly-named file fails on multiple USB sticks — it may be a bad download from a busy server.
- The official page was mid-update and briefly served an incomplete file.
- A region mismatch — a file from the wrong regional PlayStation site won’t apply.

Where exactly does the file go on the USB drive?
PS4/UPDATE/PS4UPDATE.PUP — a PS4 folder, an UPDATE folder inside it, and the file named PS4UPDATE.PUP. Any other path won’t be found.
FAT32 or exFAT?
Both work on current firmware. FAT32 is the safest default; if your file is over 4 GB (the reinstall file), use exFAT since FAT32 can’t hold files that large.
Did I download the wrong file?
Quite possibly. The update file and the reinstallation file are both .PUP but very different sizes. Using the reinstall file by mistake — or a file from another region — triggers this error.
