Ordered easiest-first. Try the next step only if the error comes back.

  1. Use the right file for what you’re doing

    Reinstalling in Safe Mode (option 7) needs the larger reinstallation file; a normal Safe Mode update (option 3) needs the smaller update file. Mixing them up triggers this error.

  2. Format the USB correctly

    Format as FAT32 (or exFAT for the large reinstall file), then create PS4/UPDATE and save the file as PS4UPDATE.PUP — the path must be exactly PS4/UPDATE/PS4UPDATE.PUP.

  3. Re-download from playstation.com

    Grab a fresh copy of the correct official file and let it download completely before copying it across.

  4. Let the copy finish, then eject safely

    A half-written .PUP is the usual cause — wait for the copy to complete fully before removing the stick.

  5. Try a different USB drive or port

    Some drives aren’t reliably detected. A small, plainly formatted stick in a rear USB port is the safest bet.
